Wolves skipper Karl Henry and central defender Jody Craddock will return to the starting line-up after lengthy injury lay-offs for the FA Cup fourth round replay with Doncaster at Molineux.
Midfielder Henry made his comeback after a two-month lay-off with a knee injury as a substitute against Manchester City at Eastlands on Saturday. Craddock has been sidelined with a hip problem since mid-October.
Midfielder David Edwards is still unavailable with a hamstring injury and full-back George Elokobi serves the second game of a three-match ban.
McCarthy accepts staying in the Premier League is the priority but wants to remain in the cup and build on Wolves' recent revival in terms of performances and results.
He said: "We want to win. We all know getting into and staying in the Premier League is worth so much money to the clubs that it has diluted the FA Cup.
"There is no question about that but it is a game of football and I'd love to still be in the FA Cup on Wednesday morning. I'm not giving that up lightly. It is funny in the early rounds, people want to protect a few players and are mindful of big games coming up.
"That is whether you are a newly-promoted team or one that wants to stay in the league, or win the Premier League. Later on, it gets a bit more important because it is still the best knockout trophy without any shadow of a doubt. This is still the best one."
Doncaster will be without new loan signing Matt Kilgallon. Kilgallon made his debut on Saturday in the home defeat to Reading after arriving on loan from Sunderland until the end of the season.
Another on-loan defender, Southampton's Joseph Mills, is doubtful due to a knee injury that forced him out of Saturday's 3-0 defeat. If Mills fails to make it George Friend will make his first start since the win at Scunthorpe in mid-October, while O'Driscoll could be tempted to recall right-back James Chambers to his back line and revert to a 4-4-2 formation.
Otherwise, O'Driscoll has no other new injury or suspension problems as his side bid to reach the fourth round for the third successive year.
